Our goal is to provide community based, long term housing with supportive
services for homeless individuals, maximizing each resident’s
ability to live independently in an appropriate housing environment.
The following information about each prospective resident will be
requested from the referral source
- New York/New York status approval
- DHS or Community referral
- Current psychosocial
- Psychiatric evaluation completed in the last six months
- Current physical exam report
- Information/evidence of any other physical disabilities
- Ability to meet criteria for DSS/SSI/VA entitlement or evidence
of other documented income, e.g. public assistance
The selection for placement into the permanent housing program
includes an assessment of an individual’s demonstrated and
consistent ability to function in the following areas with minimum
supervision for the last six months, including:
- Self medication, if medication is
necessary
- Menu planning
- Money management and budget planning
- Household cleaning and cooking
- Ability to communicate needs
- Ability to shop for food, clothing and household items
- Ability to follow directions
- Self direction and goal setting skills
- Ability to keep appointments
- Willingness to participate in activities which enhance independent
living skills.
Individuals referred, who have previously demonstrated violent
behavior directed at themselves or others, will be considered on
an individual basis. Those with active alcohol or drug abuse problems
will be considered for housing and will be required to actively
participate in their individual substance abuse treatment plan.
A screening team consisting of the Director of Clinical Services,
the Project Director, and a Senior Case Manager will review material
for each prospective resident. An on-site interview will be scheduled.
In the event of a rejection or decision to defer, a written explanation
will be provided to the referring agency in ten business days or
less. This will include short term objectives needed in order for
the candidate to be admitted when appropriate.
